What partnership looks like

The first 90 days, line by line.

Phoenix is at the start of its LTC pharmacy operation and onboarding a first cohort of partner facilities right now. Below is the exact 14-day-onboarding-then-90-day-performance commitment we put in writing for every new partner.

A note from the pharmacy: we're at the start of the LTC partner book. Rather than publish anonymized numbers we don't yet own, this page documents what we commit to in writing. The first partner outcomes — STAT delivery medians from eMAR timestamps, F-tag prevention, med-error reductions, and DON references — will be published here in Q4 2026, with each partner's permission. Days 0 – 14

Onboarding without resident disruption.

  1. Days 1–3. Census capture, insurance verification, allergy/diagnosis cross-check, transfer of every active prior auth.
  2. Days 4–7. Packaging configuration set to your preference (color-coded by shift / by resident, blister vs single-dose pouch), eMAR integration verified end-to-end (PointClickCare).
  3. Days 8–11. Staff training on med pass, label conventions, controlled-substance count workflow, after-hours access, and STAT delivery procedure. We bring food.
  4. Days 12–14. First cycle fill on your existing schedule (no MAR cycle reset), first DRR delivered to your medical director, first monthly visit booked with your DON.

Days 14 – 90

What we commit to in writing.

  • Every STAT order verified by a pharmacist within 30 minutes and delivered within 4 hours of eMAR order entry. Comfort kits at midnight, behavioral PRNs on weekends, C-II with two-person witness signature. Median delivery time reported to your DON every month.
  • Monthly on-site DRR by a named consultant pharmacist. Same pharmacist every visit, direct cell on file with your DON, written recommendations to the medical director within 5 business days.
  • Title 22 / §87468.2 documentation matching CDPH expectations. Storage logs, polypharmacy assessments, GDR attempts, psychotropic informed-consent chain — ready before the surveyor walks in.
  • After-hours pharmacist on call — direct line, not a queue. Nights, weekends, holidays. The same pharmacist your DON met during onboarding.
  • If we miss any of the above, you switch back at no cost. We handle the reverse transition. No restocking fees, no awkward conversation, no resident disruption. That commitment is in the contract.

What we measure

The dashboard your DON will see at month 3.

  • Median STAT delivery time, with the long tail flagged for root-cause review
  • Med-pass error reportables, by category and by shift
  • Polypharmacy load (residents on 9+ medications) with quarterly delta
  • GDR attempts, by class, with physician acceptance rate
  • F-tag risk surface — F755, F757, F758, F759, F760, F761 — auto-flagged from chart spot-checks
  • Cost per resident, by therapeutic class, vs. prior 12-month baseline if available
